8オンライン診療における個人情報の取扱
当院がオンライン診療を提供する際、以下の個人情報を取扱います。
- ビデオ通話における映像・音声（録画は患者同意がある場合のみ）
- 診療セッション中に医師が記録する診療メモ（医師法第 24 条第 2 項に基づき 5 年保存）
- 本人確認のためにアップロードいただく身分証画像（健康保険証・マイナンバーカード・パスポート等）
- 診療に関する同意の取得記録（同意書ハッシュ値・電子署名画像・取得日時・IP アドレス）
当院は、厚生労働省「オンライン診療の適切な実施に関する指針」を参照し、 本人確認、同意取得および安全管理措置等の運用状況を必要に応じて確認・見直します。
9業務委託先 / 利用する第三者サービス
当院は、サービス提供のため以下の第三者サービスを利用します。 実際の処理範囲、処理地域、保存期間および安全管理措置は、診療所が利用する契約・設定に応じて確認します。
- Supabase（データベース、認証およびファイル保管）
- Vercel（Web アプリケーションの配信およびサーバー処理）
- Twilio（ビデオ通信）
- Stripe Japan（決済処理）
- Resend（メール配信）
- LINE Platform（LINE Login および Messaging API）
- OpenAI（多言語応答、音声文字起こしおよび画像解析のための AI 処理、米国）
Text, audio, and images that you send through LINE are sent to OpenAI in the United States for multilingual replies, booking assistance, audio transcription, and image analysis only after you send the specified explicit consent command in LINE and while that consent record remains valid. These contents are not sent to OpenAI before consent, if you decline, or after you withdraw consent.
Actual retention and whether data may be used for model training depend on the clinic's OpenAI API contract and configuration. The clinic operator must verify and continue to manage the contract and configuration actually in use.

11患者様の権利
個人情報保護法および医療法に基づき、患者様は以下の権利を有します。
- 開示請求権 — 当院が保有する患者様の個人情報の開示を求めることができます
- 訂正・追加請求権 — 個人情報の内容が事実と異なる場合、訂正を求めることができます
- 利用停止・削除請求権 — 法令で定める場合を除き、個人情報の利用停止を求めることができます
- 同意の撤回 — 録画・データ共有等の同意は、いつでも撤回できます（医師法による保存義務の範囲内で対応）
